Dementia associated with Parkinson disease (PDD) occurs in more than 75% of patients over the disease course1 and has devastating consequences for patients, families, and society.2 Postmortem studies have shown that the pathologic substrate for PDD is heterogeneous, composed to varying degrees of cortical Lewy bodies, diffuse and neuritic amyloid plaque pathology, and neurofibrillary tangles.3,4 Biomarkers for PDD that are relevant to its underlying pathology could offer a glimpse into the heterogeneity underlying PDD and enable development of specific treatments for this highly disabling problem.
