Editors' Note: In China, the most common reason for eligible patients with acute strokes to not receive tissue plasminogen activator is patient or family refusal, while in the United States, it is mild or resolving symptoms. Hu et al. and article authors Messé and Schwamm discuss possible reasons for this difference. In reference to “Therapeutic coma for status epilepticus: Differing practices in a prospective multicenter study,” Dr. Sethi states that since the study showed that therapeutic coma in status epilepticus resulted in an increased length of stay and associated costs with no change in mortality, the recommendation to follow published evidence-based guidelines in the treatment of status epilepticus should remain in place, with the use of therapeutic coma considered on a case-by-case basis.
