Diffusion-weighted imaging (DWI) has revolutionized stroke diagnosis with its ability to detect small areas of brain ischemia within 30 minutes of onset.1 Its application in ischemic stroke and TIA has provided insights into etiology and prognosis.2 Following intracerebral hemorrhage (ICH), the occurrence of ischemia has been sought for decades, focused primarily in the peri-hematomal region.
