Throughout 2007 my editorial messages have largely focused on communicating the current status and future direction for the Journal. In this piece I will summarize our performance in 2007 and our plans for the coming year.
Early on, we outlined the immediate changes to the Journal.1 These changes included our decisions to publish weekly and to concentrate our advertisements in the front matter of the Journal paired with abstracts from recent issues to provide approximately 90 pages of uninterrupted editorial material in the print journal. We adopted a new design focusing, in part, on a more reader-friendly presentation of illustrations and began publishing an increasing amount of supplemental material online. We developed mission and vision statements to guide our decisions, revised the review process to speed decision-making on submitted manuscripts, and committed to publish a greater percentage of high-impact papers online ahead of print (“e-Pub”). We promised to reduce the acceptance-to-publication lag2 (note: we have not yet achieved this goal; see below). In July we published our strategic plan; this plan has been useful to measure our progress and to map “next steps.”3 In October, we joined with other biomedical journals in dedicating one issue to global health (our decision to do this in coming years will depend, in part, on input from our readers).4 We have recently launched our weekly podcast initiative.5

Submission specifications:
- Submissions must be < 200 words with < 5 references. Reference 1 must be the article on which you are commenting.
- Submissions should not have more than 5 authors. (Exception: original author replies can include all original authors of the article)
- Submit only on articles published within 6 months of issue date.
- Do not be redundant. Read any comments already posted on the article prior to submission.
- Submitted comments are subject to editing and editor review prior to posting.
