Ocular flutter is a disorder characterized by intermittent bursts of conjugate horizontal saccades without intersaccadic interval, probably caused by dysfunction of omnipause neurons in the paramedian pontine reticular formation.1,2⇓ Causes of ocular flutter include parainfectious and paraneoplastic syndromes and metabolic-toxic states.1 However, in some patients no underlying disease can be found.1 We report a case of intracranial hypertension following cerebral venous thrombosis, where persisting ocular flutter was the only detectable clinical abnormality.
Case report.
A 27-year-old engineer was seen for evaluation of headache and intermittent oscillopsia. Two years previously, he developed an acute, severe, right-sided pricking headache associated with right-sided tinnitus, nausea, and light and noise sensitivity. Several days after headache onset, he noted horizontal oscillopsia, which lasted for “fractions of a second,” and occurred up to 10 times daily. He was seen in a neurology department, where physical examination showed no abnormalities. MRI revealed thrombosis of the right transverse and sigmoid sinuses. No hypercoagulable state …
