The Ommaya reservoir facilitates repetitive sampling and delivery of drugs into the CSF. The silicone rubber capsule is biologically inert and self-seals after puncture with a small needle; however, more than 200 punctures may predispose to leakage from the capsule. Ommaya complications include infection, hemorrhage, reservoir dysfunction and malpositioning, seizure, focal deficit, and focal leukoencephalopathy.1 The most common organism to infect the reservoir is a coagulase-negative Staphylococcus from skin contamination.2,3⇓ There is no direct relationship between the number of reservoir punctures and infection, but infection usually occurs within weeks of use.2,4⇓ We report three cases of Ommaya infection with Staphylococcus species occurring years after the last puncture of the device (table).
