On a lazy Sunday afternoon this past summer I was sitting under the linden tree in my backyard listening to the radio. Carlos Fuentes, Ambassador of Mexico to the United States and one of the leading Latin American novelists, was rambling on about his life and works. He spoke on the subtle differences between progress and evolution, which struck a note. Indeed, what he was saying could be applied to neurology. Progress in the neurosciences has been phenomenal but its evolution has lagged behind, particularly as a well rounded body of clinical science in the social setting of the international arena. Neurology is a discipline for rich countries. What percentage of the world population has access to diffusion MRI, thrombolytic therapy, transcranial Doppler, plasmapheresis, gamma-knife, polysomnography, thalamic stimulation? The ability to deliver modern neurology in developing countries is lagging far behind the enormous progress in developed nations, and the divide is widening. Is anyone looking into this? What are the solutions proposed by the international bodies of neurology?
