The differential diagnosis between brain abscess and necrotic brain tumor may be difficult to establish. [1] Radiologically (CT or MRI), both lesions appear as an expansive mass surrounded by edema, with ring enhancement Figure 1, A. Clinically, brain abscess induces fever in only 50% of cases, and history of local infection may be lacking. In addition to morphologic information obtained by MRI, proton magnetic resonance spectroscopy (sup 1 H MRS) provides metabolic information about healthy and pathologic tissues.1 H MRS has been applied extensively for examining brain tumors in vivo, [2,3] but studies reporting1 H MR spectra from brain abscesses are rare. [2-6] We applied the technique to a series of patients with necrotic brain tumors and brain abscesses.
