先前健康的43岁男子提出性交的雷声头痛。脑CT扫描显示perimesencephalic蛛网膜下腔出血(PMSAH)。数字减影血管造影(DSA)表现出严重的基部的动脉血管痉挛没有底层动脉瘤(图1)。nimodipine治疗下,血管痉挛逐步解决。个月随访,高分辨率(3 t) MRI (HR-MRI)发现了一个原始左基底静脉罗森塔尔(BVR)排水侧中脑的静脉而不是方铅矿的系统。1
大约一半的nonaneurysmal PMSAH与原始的引流静脉变异(图2)。2这些变异与血流动力学扰动,导致逆行流,突然压力变化和静脉破裂,导致PMSAH。2患者BVR港4倍增加的风险nonaneurysmal PMSAH与动脉瘤性SAH相比。1DSA的频繁不可用静脉阶段,评估应该考虑HR-MRI原始BVR和底层PMSAH静脉异常。在患者典型的PMSAH,识别的原始BVR HR-MRI提供了一个底层etiopathogenetic机制可能呈现一个重复DSA冗余。1
